　　I certify I have complete understand the Service knowledge, and now I confirm the information which I’m providing here in this application is true and complete, and confirm that this BSCI audit to be carried out by SGS is per the request or confirmation from BSCI member _________(* One is enough).

　　我方已完全知悉审核服务须知，并因此保证此申请表中我方提供的资料是真实完整的，并且确认此次BSCI审核已在BSCI会员(仅需填写一个)_________确认并同意的条件下向SGS公司提出审核申请。

　　Service Knowledge(审核服务须知)

　　1, BSCI audit should be initiated by BSCI member based on BSCI rule;

　　依据BSCI的要求，BSCI审核必须由BSCI的成员确认方可由认可的审核公司以实施;

　　2, Factory should bear the corresponding risk if the provided information is spurious: the false declaration might

　　lead to the withdrawing of the BSCI audit report from the customer list of this BSCI member and BSCI

　　database.

　　如果被审核工厂所提供的信息是不真实，那么被审核工厂需要承担因此产生的风险和责任, 例如：该报告将不会被此BSCI会员承认，同时该报告也会从BSCI数据库中删除

　　根据申请表的内容，工厂若提供不实的客户信息，包括客户的会员号不正确，或者客户向BSCI官方投诉该工厂并非其供应商，则BSCI审核报告将从数据库下架。
